5.3. The Services are provided in accordance with the Patient's Informed voluntary consent to diagnostics, treatment, surgery and anesthesia, which is executed in the form established by the legislation of Ukraine and may additionally be executed in a form (questionnaire) determined by the Provider, before the first Service is provided and before other Services, the list of which is determined by the Provider. The Parties have agreed that signing the informed consents and/or questionnaires is a necessary condition for starting the provision of the Services.

5.4. The Services are provided during the Provider's working hours. The Services are provided by prior appointment, made by phone or when the Patient applies in person. Services may be provided without a prior appointment only if no other Patients have an appointment for that time. The date and time of each Service are agreed by the Provider and the Patient orally or in writing (by signing a visit schedule or in another informative form).

5.5. The date and time of each Service may be changed at the Patient's initiative before the scheduled time of such Service.

5.6. If the Patient is running late, they must notify the administrator of the Provider's dental office in advance. If being late has shifted the appointment schedule, another visit time is agreed with the Patient.

5.7. The date and time of each Service may be changed at the Provider's initiative in the following cases:

5.10. If the Patient gave notice of cancelling an online consultation less than 24 hours in advance, the deposit in the amount of the service cost is not refunded; at the Patient's request, the service may be rescheduled to another day according to the Provider's schedule.

5.11. Payment for a consultation already received online or offline is not refundable.

5.12. When making a prior appointment for treatment or a consultation, the patient pays a deposit of 500 UAH; if the patient gives notice of cancelling the visit later than 24 hours before the appointment, the deposit is not refunded.

6. Cost of services and payment procedure

6.1. The cost of the Services provided under this Agreement is determined according to their scope and the rates for the Services approved by the Provider. The rates for the Services have the legal force of an agreement on an agreed price.

6.2. The preliminary cost of the Services is specified in the Treatment plan and does not include the cost of treating hidden pathologies that may be detected during treatment. After the Treatment plan has been agreed, the cost of general, surgical and orthodontic treatment may change due to hidden pathologies or other circumstances, of which the patient will be notified in advance. If additional services affect the overall result of treatment under the Treatment plan and the Patient refuses them, such Patient may be refused further treatment at the facility, and funds for services not received will be refunded in the case of prepayment.

6.3. The prices specified in the Treatment plan are valid for the period specified in the Treatment plan. After this period expires, the cost of the Services is determined based on the Provider's rates in effect at the time the Service is provided.

6.4. The Patient pays for the Services in one of the following ways at the Patient's choice:

6.4.1. payment in cash;

6.4.2. payment of the Provider's invoices by bank transfer.

6.5. The Patient pays for the Services on the day the Service is provided, before the Service or immediately after it, in the amount of the full cost of the Service provided at the specific visit.

6.6. The Patient has the right to prepay the Services defined by the Treatment plan. The cost of the Services prepaid by the Patient cannot be revised by the Provider if the rates for the Services change, and is set according to the rates in effect on the day of payment.

6.7. For certain types of Services that involve preliminary expenses of the Provider (individual orders of treatment appliances for the Patient from third parties, etc.), the Patient must make a prepayment of 50 to 70% of the cost of such appliances. The prepayment amount and its due date are specified in the Treatment plan and/or in the invoice provided to the Patient in advance.

6.8. Services paid by bank transfer are provided only subject to 100 % prepayment.

6.9. If the total cost of all components of the prepaid Services exceeds the prepayment amount made by the Patient, the Patient must pay the difference no later than the day the Services are completed.

6.10. If the amount of the invoice issued to the Patient at a specific visit exceeds the estimated cost of the Service (part of the Service), the Patient must pay the invoice no later than the day after the Service (part of the Services) is provided. In this case, the Patient must sign a Certificate of services rendered listing the Services provided and stating the amount of the Patient's debt for the Services.

6.11. If the Patient does not have sufficient funds to pay for the Services received, the Patient's debt to the Provider may be formalized by an additional agreement on payment for the Services in installments.

6.12. Treatment under insurance is provided subject to the Provider having previously concluded a relevant Agreement with the insurance company.

6.13. The Patient may be granted a discount on the cost of a Service, the amount of which is determined in the manner established by the Provider.

6.14. If during the provision of the Services they need to be adjusted (additional services provided or the Treatment plan changed), the final cost of the Services under the Agreement is adjusted accordingly. The Parties agree on such changes before the additional or changed Services are provided. The Patient has the right to refuse the adjustment of the Services and continue to receive the Services under the agreed Treatment plan. If such refusal makes it impossible to continue providing the Services for medical reasons, the Provider has the right to terminate this Agreement unilaterally, and the Patient must pay the cost of the Services actually received. If the Patient does not object to the change or addition to the Treatment plan, the provision of the Services continues under the new terms with a new or additional Treatment plan signed.

6.15. If through the Provider's fault the Services are not provided in full, the cost of the Services paid but not provided is refunded to the Patient within three working days from the date the Patient submits a relevant application.

6.16. If the Services (part of the Services) cannot be provided because the Patient failed to attend an appointment with the doctor or another medical procedure, refused to receive further Services, or violated the terms of this Agreement and/or the Rules, the Provider shall refund to the Patient, within three working days from the date the Patient submits a relevant application, the prepayment amount paid, less the expenses actually incurred and the expenses of manufacturing (ordering) treatment appliances for the Patient.

6.17. The Patient has no right to refuse to pay for Services that were agreed and actually provided.

6.18. In all cases where the cost of the Services provided to the Patient is not directly determined by the Treatment plan, including where the Services defined in the Treatment plan were not provided in full, the cost of such Services (part of the Services) is determined according to the Provider's current rates.

6.19. The total cost of the Services under this Agreement is determined as the sum of the cost of all Services provided to the Patient.

6.20. The Patient pays a deposit for the manufacture of orthodontic or prosthetic appliances in the amount of 50% of the preliminary cost of treatment; the deposit is not refunded if the service was not provided through no fault of the Provider or if the Patient refused further treatment.

10. Implants

10.1. Implant placement surgery is carried out in two stages:

  • Stage 1: placement of the intraosseous part of the implant;
  • Stage 2: (approximately 3 - 6 months later) placement of a healing abutment, and then, 10- 20 days later, of the abutment.

10.2. Implant placement is a less traumatic procedure than tooth extraction. During the surgery, which is performed under anesthesia, Patients experience almost no discomfort.

10.3. The main advantages of implants over conventional prosthetic methods are:

  • solving the problem of poor retention of removable dentures (a full removable denture can be fixed on 2 or 4 implants) or doing away with removable dentures entirely (when a sufficient number of implants is placed);
  • the possibility of keeping the teeth adjacent to a gap in the dentition intact, as well as achieving adequate functional load and preventing overload of these teeth;
  • if necessary, the outer (crown) part of the implant can be replaced.

10.4. Besides advantages, implants also have disadvantages:

a) the need for surgery;

b) lengthy and complex treatment;

c) possible serious complications;

d) the long-term result of treatment and the cosmetic result are difficult to predict.

10.5. Possible complications include: osteomyelitis (inflammation of bone tissue), bone necrosis (death of bone), penetration (damage to, or perforation of, the maxillary sinus and the mandibular canal), increased tooth mobility, which may make extraction necessary.

After surgery, some Patients may experience gum inflammation or develop temporomandibular joint pathology during recovery, with consequences such as: headache radiating to the neck and facial muscles, fatigue of the chewing muscles, possible swelling, changes in the mucous membrane at the surgical site, numbness of the lip, tongue, cheek and teeth, slow healing, allergic reactions to the medicines used, etc.
